IHOP connects vulnerable people in interim housing to community health services by identifying mental health, substance use, and activities of daily living (ADL) needs with the end goal of having people in permanent housing. It is a collaborative effort between the the Department of Mental Health, Department of Homeless Services and Housing, the Department of Public Health, LA Care, Health Net, and other plan partners.
